Job summary

Moments Hospice is seeking an RN Case Manager to serve as the clinical lead for patients and families at end of life. You will manage symptoms, coordinate care, and provide guidance during a meaningful transition.

The role emphasizes a compassionate, patient- and family-centered approach with strong collaboration across the care team, limited caseloads, and access to leadership for growth. Fleet vehicle program supports mobility and patient care.

Qualifications

• Active RN license in the state you work in.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
