Job summary

Spirit Hospice of Georgia is seeking a Nursing Case Manager to provide compassionate hospice care, coordinating with families and the care team to ensure comfort and informed choices. You will be the clinical anchor, supporting patients at every step with empathy and clear communication.

The role emphasizes teamwork, dependable transportation, and ongoing patient advocacy within a locally owned hospice environment that values people over process.

Qualifications

• Current RN license in the State of Georgia.
• CPR and First Aid certification required.
• Reliable and legal transportation to patient residences.
• Hospice or home care experience is a plus; kindness and reliability matter most.

Responsibilities

• Serve as the clinical anchor for patients and families, ensuring support, information, and compassionate care.
• Collaborate with an interdisciplinary team to coordinate end-of-life care.
• Advocate for patient comfort, dignity, and informed decision-making throughout the care plan.

Position Requirements:

• Current RN license in the State of Georgia
