Job summary

Spirit Hospice of Georgia is seeking a dedicated RN Case Manager to serve as the clinical anchor for patients and families across our local hospice program. You will collaborate with a team to deliver compassionate end-of-life care, ensuring patients feel supported, informed, and genuinely cared for at every step.

The position emphasizes teamwork, empathy, and flexibility, with a focus on delivering high‑quality home hospice services within the Georgia community.

Qualifications

• Current RN license in the State of Georgia.
• Current CPR & First Aid Certification.
• Reliable and legal transportation to and from patient's residence.
• Hospice or home care experience is a plus, but kindness and reliability matter most.

Responsibilities

• Be the clinical anchor for patients and families, providing compassionate end‑of‑life care.
• Coordinate care with an MDT to ensure informed, supported patients and families.
• Advise, educate, and support families throughout the patient journey.

Position Requirements:
