Job summary

Hospice of the West seeks a compassionate Registered Nurse Case Manager (RNCM) to join our nursing team in Phoenix. This PRN role delivers hands-on hospice care, coordinates services, and supports patients and families throughout the hospice journey.

You will work with our interdisciplinary team, providing comfort and clinical excellence, while guiding care plans and education to families. Requires current RN license and at least 1 year in med-surg/acute care; hospice experience is a plus.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with computers and EMR systems.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Deliver direct hospice nursing care and coordinate services.

Hospice of the West is seeking a PRN Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: PRN, as needed

Territory: Phoenix and surrounding cities

Qualifications

• Hold current unencumbered license as a Registered Nurse.


• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!


• Must be computer proficient in typing and various programs, including background in EMR.


• Possess and maintains current CPR certification if required by state.


• Current driver's license, vehicle insurance, and access to dependable transportation.

$21.50 - $24.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
