Job summary

Santa Clara County Health Plan is seeking a Temp LVN Case Manager in San Jose to support SNP care coordination under RN supervision. The role focuses on data collection, ICP drafting, and SBAR summaries, with productivity tracked daily.

The position requires a California LVN license and 1 year in care coordination or ambulatory care, with strong attention to detail and meticulous documentation.

Qualifications

• Current California LVN license in good standing.
• Minimum 1 year of experience in care coordination, ambulatory care, or managed care.
• Experience with chart review and care plan implementation preferred.
• Strong attention to detail and ability to follow workflows.
• Excellent documentation accuracy and communication skills.

Responsibilities

• Conduct focused data collection and basic assessments for assigned member cases.
• Review clinical records, medication lists, encounter history, and care plan status.
• Draft ICP components based on outreach and member needs, under RN guidance.
• Prepare SBAR summaries for RN review and finalization.
• Report any significant changes or redflag findings to the supervising RN.
• Maintain accurate, timely documentation in accordance with SNP requirements.
• Support care coordination outreach and followup tasks within LVN scope.
• Productivity monitored daily.
