Qualifications

• Current California RN license.
• Current CPR certification.
• Two years’ experience with direct patient care and coordination preferred.
• Fluency in English and Spanish preferred.

Responsibilities

• Assists with meeting the needs of complex medical patients, including assessment, education, referrals, and follow-up care.
• Provides nursing care including administration of medications and treatments per orders, policies, and CA guidelines; EMR documentation.
• Collaborates with the clinical team to improve quality of patient care and infection control.
• Interprets patient information and documents care in the EMR.
• Provides complex care coordination including triage, phone follow-ups, refills, labs, and case management.
• Monitors tracking systems for abnormal labs, infectious diseases, and high-risk referrals.
• Guides LVNs and other team members in health care delivery and coordination.
• Develops independent care plans for chronic disease management.
• Maintains skills proficiency and participates in ongoing training.
• Understands CDC/VFC immunization schedules, storage, and handling.
