Loving Care In-Home Health Services in Yukon, OK is seeking an experienced Registered Nurse Case Manager for our Yukon Home Health Office. This full-time day-shift position involves supervising care, coordinating with physicians and nursing staff, and ensuring high-quality patient outcomes.

You will document visits using EMR systems and must have a current Oklahoma RN license, at least 1 year of home health or acute-care experience, ability to lift 20 lbs, a vehicle with insurance, and a phone

Qualifications

• Current Registered Nursing License approved by the Oklahoma Nursing Board of Regents.
• Minimum 1 year experience in home health or acute-care with elderly, middle-aged and/or pediatric care.
• Able to lift a minimum of 20 lbs without difficulty.
• Must own a vehicle in which to make visits and maintain current automobile insurance coverage.
• Must have a telephone available for after-hours calls when needed.

Responsibilities

• Supervise and manage all care given to assigned case management patients.
• Oversee nursing staff that are assigned to patients as ordered by physician.
• Coordinate work schedule to fit patient's time and needs.
• Complete patient visit documentation and submit electronically using WellSky (Kinnser) EMR system.
