Qualifications

• Active RN license in Oklahoma (in good standing).
• At least 2 years of home health nursing experience.
• CPR certification (or ability to obtain within 60 days).
• Knowledge of Medicare home health guidelines.
• Valid driver's license, reliable transportation, and insurance.

Responsibilities

• Support the Clinical Manager in overseeing daily clinical operations.
• Assist with the development and revision of individualized patient care plans.
• Respond promptly to staff and physician calls regarding orders or care concerns.
• Lead weekly case conferences and support decisions based on clinical findings.
• Maintain strong referral source relationships and promote positive patient outcomes.
• Assist with scheduling and occasionally provide direct patient care.
