Job summary

Comfort Keepers is seeking an experienced Registered Nurse (RN) Case Manager to join our team. This field-based role requires travel to client homes across Oklahoma and offers flexible hours with Part-Time/PRN or Contract options.

You will perform in-home assessments, develop care plans, supervise caregiver activities, and maintain accurate electronic documentation. Ideal candidates hold an active RN license, have 3+ years in home health or related fields, and can travel throughout the service

Qualifications

• Active and unrestricted RN license required.
• Minimum of three years in Home Health, Hospice, Rehabilitation, or Private Duty Nursing.
• Valid driver’s license and reliable transportation; ability to travel to client homes.
• Strong computer skills and electronic documentation experience.

Responsibilities

• Conduct intake calls and in-home assessments to develop individualized care plans.
• Coordinate care with clients, families, physicians, and agency staff to ensure continuity.
• Perform supervisory visits; monitor caregiver performance and coaching needs.
• Ensure compliance with policies, HIPAA, and quality assurance programs.
• Document assessments, care plans, medication profiles, and visits timely.
