Amedisys Home Health in the United States is seeking an experienced Registered Nurse to join our home health care team as a case manager. You will assess patients, coordinate with the care team, and implement personalized plans for safe, effective recovery at home.

We offer a $20,000 sign-on bonus, competitive annual pay ranging from $74,802 to $93,506.40, a full-time day schedule, robust benefits, and opportunities for professional growth within a trusted national home health provider.

Formación

• Minimum 1 year RN clinical experience.
• Current RN license in state(s) served.
• Current CPR certification.
• Valid driver’s license and liability insurance.

Responsabilidades

• Performs patient assessments and works with the care team to develop a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitation measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
