Job summary

Amedisys is seeking an experienced RN Case Manager for Home Health. This full-time daytime role involves patient assessments, care planning, and coordination with the care team to improve outcomes.

You will supervise LPNs and HHAs, complete timely documentation, and participate in on-call duties as needed. An active RN license and CPR certification are required, along with reliable transportation.

Qualifications

• Registered Nurse with 1+ year clinical RN experience (or VP approval if less).
• Current RN license for the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• RN Intern program if less than 6 months RN experience.

Responsibilities

• Assess patients and collaborate with the care team to plan and implement care.
• Refer to other disciplines as indicated and document rationale.
• Promote health and independence through teaching and rehabilitative measures.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Communicate patient progress to clinical manager and care team.
• Plan and provide staff education.
• Perform on-call duties and services as assigned.
• Participate in clinical development and continuing education.
