Job summary

ExcelaCare Home Health is seeking an experienced Registered Nurse to administer home health care across patients’ homes and facilities. You will coordinate care with a multidisciplinary team and assume responsibility for the Plan of Care, working independently without on-site supervision.

Ideal candidates have a nursing degree, 2 years RN experience (home health preferred), an active state RN license, CPR, and a valid driver’s license.

Qualifications

• Graduate of an accredited school of nursing.
• Two years RN experience, preferably in home health.
• Current valid State Nursing license (RN), CPR certification.
• Driver’s license and proof of auto liability insurance; no OIG Excluded Provider listing.

Responsibilities

• Apply nursing knowledge to plan and execute patient care.
• Perform baseline nursing skills per agency policy and standards.
• Administer medications and treatments per physician orders.
• Develop and document Plan of Care with the patient.
• Document patient status accurately and timely.
• Coordinate with referring physicians and families.
• Collaborate with interdisciplinary team on care plan.
• Educate patients and families on health care plan and end-of-life options.
• Attend on-call rotation and make visits as scheduled.
• Maintain confidentiality and comply with laws and agency policies.
• Travel locally to patient homes and facilities.
