Job summary

ExcelaCare is seeking a Registered Nurse to deliver skilled nursing care in patients' homes, coordinating with a multidisciplinary team and families. The role requires independent practice with adherence to policy and plan of care.

You will administer treatments, document progress, educate patients, and participate in ongoing education while maintaining compassionate, patient-centered care in diverse home environments.

Qualifications

• Graduate of an accredited nursing program.
• One year of nursing experience.
• Current State RN license; CPR; valid driver's license and auto liability proof.

Responsibilities

• Administer skilled nursing care to patients in their homes across ages.
• Coordinate care with multidisciplinary team, patient/family, and referring agency; work independently.
• Perform nursing assessments, develop and implement Plan of Care per physician orders.
• Document patient status, interventions and education clearly and timely.
• Provide supervision and teaching to LVN/LPN and home health aides as needed.
• Attend case conferences and participate in on-call rotation.
