AccentCare, Inc. is seeking an RN Case Manager for Home Health in Minnesota. The role oversees care plans from admission to discharge, coordinating nurses and ALH clinicians to ensure high-quality patient care.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as an RN in the state of operation
• A minimum of one year of experience as an RN
• Current CPR Certification required

Responsibilities

• Assess the status of nursing needs of assigned patients and overall management of the patient's care plan
• Complete initial patient assessment upon admission, including appropriate patient history
• Select appropriate nursing diagnosis
• Assess all medicines to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contraindicated medications
• Establish realistic, measurable, observable goals consistent with the patient's diagnosis
• Inform the physician of the results of the assessment and recommended clinical interventions
• Implement appropriate nursing interventions consistent with the patient's diagnosis and established goals and within the scope of the Nurse Practice Act
• Re-evaluate patients' nursing and other clinical needs as required
• Document and submit the assessment, progress, and discharge notes and other required paperwork in a timely fashion as required by the agency
• Inform the physician and other personnel of changes in the patient's condition and needs
