AccentCare, Inc. is seeking an RN Case Manager for home health in Minnesota, covering Burnsville and surrounding areas. Schedule is Mon-Fri, 8:00 a.m. – 5:00 p.m.

with a $5,000 sign-on bonus. Pay ranges from $87,000 to $97,000 annually and is noted as an estimate, paid on a per-point basis. This role manages care plans from admission through discharge, directing staff and coordinating with clinicians to ensure quality patient care.

Qualifications

• Must be a graduate from an approved school of professional nursing.
• Active RN license to practice in Minnesota.
• Minimum of 1 year of RN experience.
• Current CPR Certification required.

Responsibilities

• Assess status of nursing needs and manage the patient’s care plan.
• Complete initial patient assessment upon admission with patient history.
• Select appropriate nursing diagnoses.
• Assess all medications for adverse reactions and contraindications.
• Establish measurable goals aligned with diagnosis.
• Inform physician of assessment results and recommended interventions.
• Implement nursing interventions per diagnosis and goals and within Nurse Practice Act.
• Re-evaluate patients’ nursing and clinical needs as required.
• Document and submit assessment, progress, discharge notes timely.
• Inform physician and staff of changes in patient condition.
