Victory Home Health - Durant is seeking a Registered Nurse Case Manager for PRN per-visit assignments. You will administer home health services, coordinate care with a multidisciplinary team, and ensure adherence to plans of care in patients’ homes or facilities.

The role requires a nursing degree, current RN license, CPR certification, and the ability to travel locally. You will document patient status, educate families, and participate in on-call rotations as needed.

Qualifications

• Graduate of an accredited nursing program.
• Two (2) years RN experience in a clinical setting preferred home health.
• Current valid State RN license and CPR certification.
• Driver’s license and auto liability insurance; no OIG exclusion.

Responsabilités

• Administers home health care for clients in their homes or facility settings per physician orders.
• Develops and implements a written Plan of Care with the patient and family.
• Documents patient status accurately and timely; communicates with interdisciplinary team.
• Coordinates care with referring physicians and community agencies; participates in on-call rotation.
