Job summary

Visiting Nurse Health System is seeking an RN Case Manager for Home Health who coordinates care for a caseload of patients, ensuring care meets quality standards and utilization guidelines. The RN Case Manager assesses, plans, implements, monitors, and evaluates comprehensive care plans to promote optimal patient outcomes in a home setting.

The position requires a compassionate nurse who values patient education and family engagement.

Qualifications

• RN license in Georgia and CPR certification required.
• Experience in home health care preferred.
• Strong assessment and care planning skills.
• Ability to educate patients and families.

Responsibilities

• Coordinate care for a caseload of patients in home health.
• Develop and update individualized plans of care with the team.
• Educate patients and families on disease management.
• Complete and submit OASIS documents and visits timely.
• Communicate changes in patient status to physicians and team members.
• Supervise LPNs and HHAs per VNHS policy.
• Participate in SOC/ROC and weekly team conferences.
• Deliver high-quality home-based nursing care.
• Maintain competencies through education.
