Job summary
Nightingale Home Healthcare in Minnetonka, MN is expanding its RN team with full-time Registered Nurse Case Managers. This PAY PER VISIT role offers flexible scheduling and a path to high earnings based on visits.
Duties include obtaining patient history, performing exams, developing care plans, coordinating care with LPNs and aides, and ensuring HIPAA compliance. Requires MN RN license, 1 year med-surg/home health, WellSky experience preferred, TB and CPR current, and a 3-day orientation.
Qualifications
- Licensed Registered Nurse in MN.
- Minimum 1 year medical/surgical environment, Home Health (OASIS) or geriatric nursing experience preferred.
- WellSky (Kinnser) experience preferred.
- Current TB results (within last 12 months).
- Current CPR Certification.
Responsibilities
- Obtains medical history and conducts brief physical examination.
- Evaluates patient and home situation to determine required health teaching.
- Plans and provides skilled nursing care per physician orders.
- Coordinates care with LPN, Home Health Aide, and Personal Care Attendant.
- Maintains HIPAA confidentiality and documents patient notes and visits.
- Participates in interdisciplinary team conferences.
