Job summary

AccentCare is seeking an RN Case Manager for Home Health in the Waco area to oversee patient care plans from admission through discharge, ensuring the delivery of high-quality care and outcomes. This role blends direct patient contact with coordination of nursing staff to implement individualized plans.

It is a full-time, Monday–Friday position with no remote work, covering Bosque and Hamilton County. The compensation ranges from $83,000 to $102,000 annually, with per-point consideration and

Qualifications

• Must be licensed registered nurse in the state of operation.
• Minimum of one year of experience as an RN.
• Current CPR certification required.

Responsibilities

• Assess nursing needs and manage the patient's care plan.
• Complete initial patient assessment upon admission, including appropriate patient history.
• Selects appropriate nursing diagnosis.
• Assesses all medicines to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contraindicated medications.
• Establishes realistic, measurable, observable goals consistent with the patient's diagnosis.
• Informs the physician of the results of the assessment and recommended clinical interventions.
• Implements appropriate nursing interventions consistent with the patient's diagnosis and established goals and within the scope of the Nurse Practice Act.
• Re-evaluates patients' nursing and other clinical needs as required.
• Documents and submits the assessment, progress, and discharge notes and other required paperwork in a timely fashion as required by the agency.
• Informs the physician and other personnel of changes in the patient's condition and needs
