Job summary

Mission Healthcare, a leading home health and hospice provider in the western United States, seeks an RN to assess patients in their homes within the East Valley territory (Maricopa). You will develop and adjust plans of care, perform nursing procedures, and counsel patients and families to meet goals.

Requires active RN license, 1 year acute care experience, BLS/CPR, and a driving license. We offer comprehensive benefits, tuition reimbursement, and relocation support to eligible colleagues.

Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• BLS/CPR certification is required.
• Valid driver's license and auto insurance.
• Associate's or Bachelor's degree in Nursing; 1 year home health experience preferred.

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates the plan of care and revises it as patient status changes.
• Regularly re-evaluates patients’ needs and status.
• Uses health data to determine nursing diagnosis and care plans.
• Develops a care plan with goals based on nursing diagnosis with patient/family input.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family on plan of care and related needs.
• Provides health instructions per assessment and plan.
• Identifies discharge planning needs and implements prior to discharge.
