Responsibilities
- Complete initial patient assessment to determine healthcare needs.
- Initiate and revise the care plan as patient needs change.
- Provide direct patient care as defined in the State Nurse Practice Act.
- Act as Case Manager coordinating care for assigned caseload.
- Participate in interdisciplinary care team meetings.
