Job summary

Mission Healthcare is seeking an RN for Per Diem/PRN home health in the East Valley Territory (Maricopa). The role involves comprehensive patient assessments, developing and revising care plans, and delivering skilled nursing services in the home setting.

The ideal candidate holds an active RN license in the state, has at least 1 year of acute care experience, and is willing to provide patient education and discharge planning.

Qualifications

• Current and valid state RN license.
• 1 year of acute care experience in an institutional setting preferred.
• BLS/CPR certification required.
• Valid driver’s license and auto insurance.
• Associate’s or Bachelor’s degree in Nursing with 1 year home health experience preferred.

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates the plan of care and revises as patient status changes.
• Regularly re-evaluates patients’ needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan with goals based on nursing diagnosis and includes patient and family.
• Initiates preventive and rehabilitative nursing procedures and administers medications as prescribed.
• Counsels the patient and family in meeting the plan of care.
• Provides health care instructions per assessment and plan.
• Identifies discharge planning needs and implements prior to discharge.
