Job summary

Hot Springs Health Program, Inc. is seeking a Home Health/Hospice Registered Nurse to case manage and deliver skilled nursing visits. You will coordinate home care services, supervise CNAs and PRN nurses, and ensure seamless communication across PT, OT, ST, CNA, and MSW teams.

The role requires an NC RN license and at least one year of RN experience in related settings. A driver’s license and reliable transportation are also required.

Qualifications

• Must have graduated from an accredited school of nursing and have a current RN license to practice in NC.
• Experience: at least one year as an RN in hospital/home care/hospice/public health setting.

Responsibilities

• Case manage and provide skilled nursing services for patient visits.
• Coordinate home care services and supervise CNAs/prn nurses in the home.
• Coordinate care between home care disciplines (PT, OT, ST, CNA, MSW).
• Timely & accurate documentation.
• Occasional weekend rotation.
• Maintain confidentiality and professionalism.
