Job summary

Angels Care Home Health is seeking an Registered Nurse Case Manager to coordinate patient home care. You will work with clinical teams and provide skilled nursing care in patients' homes across our service area.

The role requires an active RN license, valid driver’s license, and reliable transportation. One year of RN experience is preferred, with CPR and hepatitis consent/declination on file.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• The RN Case Manager coordinates all aspects of the patient’s home care.
• Interacts with other medical disciplines as needed and provides professional care to the patient.
• Performs admission visits to the home and determines eligibility for home care services.
• Develops and follows the plan of care for each patient.
