Job summary

Independence Home Health LLC is seeking a compassionate RN Case Manager in Bloomington, IN to join our growing team. You will assess patients, develop care plans, and coordinate with physicians, families, and your clinical teammates to ensure high-quality in-home care.

The ideal candidate holds an active Indiana RN license with at least two years of nursing experience, including home health, current CPR certification, and strong communication and leadership skills.

Qualifications

• Graduate of an accredited nursing program.
• Active Indiana RN license or a qualifying multistate license recognized by Indiana.
• Minimum of 2 years of nursing experience; home health experience preferred.
• Current CPR certification.
• Strong communication, leadership, organizational, and clinical skills.
• A commitment to providing exceptional patient-centered care.

Responsibilities

• Conduct patient assessments and develop individualized plans of care.
• Coordinate care with physicians, patients, families, and other healthcare professionals.
• Educate patients and caregivers on care plans, treatments, and ongoing health needs.
• Supervise and support LPNs, HHAs, and CNAs to promote quality patient care.
• Review and update plans of care as needed.
• Maintain accurate, timely, and complete clinical documentation.
• Participate in case conferences and agency quality improvement initiatives.
• Help ensure patients receive compassionate, safe, and effective care in their homes.
