Job summary

Independence Home Health LLC is seeking a compassionate RN Case Manager to join our team and grow with a company that values your work.

You will assess patients, develop care plans, coordinate with physicians and caregivers, educate families, and supervise LPNs and HHAs/CNAs to ensure high-quality care in patients' homes. Full-time and part-time opportunities are available.

Qualifications

• Graduate of an accredited nursing program.
• Active Indiana RN license (or NLC recognized); CPR certification.
• Minimum of 2 years nursing experience; home health preferred.
• Strong communication, leadership, and organizational skills.

Responsibilities

• Perform patient assessments and develop individualized care plans.
• Coordinate services with physicians, caregivers, and the care team.
• Provide patient and caregiver education and ongoing support.
