Central Vermont Home Health & Hospice in Vermont is seeking a Homecare Nurse Resident. This trainee role serves as case manager for assigned patients and is designed for new nursing graduates, or nurses with up to two years of experience.

You will develop care plans, perform nursing visits, and collaborate with an interdisciplinary team. The program includes on-call duties and supervision of LPN/LNA staff, with comprehensive benefits.

Qualifications

• Graduate of an accredited school of nursing with an associate or bachelor's degree in nursing.
• No nursing experience required.
• Current Vermont nursing license by date of employment.
• Valid Vermont driver's license.
• Ability to pass motor vehicle, criminal, adult and child abuse, and OIG checks.

Responsibilities

• Develop and coordinate goal-oriented care plans with patients, families and interdisciplinary team.
• Implement plans of care and nursing interventions to ensure high-quality home care.
• Maintain up-to-date electronic documentation for care integration.
• Meet nursing visit performance standards (OASIS accuracy, productivity, case mix, etc.).
• Participate in primary on-call, weekends and holidays as needed.
• Supervise LPN and LNA staff as required by regulations.

Who We're Seeking

We are seeking for an individualized goal-oriented Homecare Nurse Resident. This position serves as Case Manager in cases in which this position is the lead discipline. This position is designed for the new nursing graduate, the nurse with preferably 2 years' experience, or a nurse returning to the workforce. The expectation is to achieve ability to be proficient in the essential functions of this position within 6 months of being hired in the trainee position.

Your Responsibilities

• Develops, communicates, and coordinates a goal-oriented plan of care, including LNA care plan as appropriate, for each assigned patient in collaboration with patient, physician or allowed practitioner, and family, as part of an interdisciplinary healthcare team in accordance with State and Federal regulations, and CVHHH policies and procedures.
• Implement plans of care, nursing interventions, patient/family teaching and other appropriate procedures as necessary to ensure high-quality nursing care of the home care patient.
• Maintains up-to-date and accurate electronic documentation of care provided to ensure the integration of information for use by the heath care team.
• Meets established nursing visit performance standards to include, but not limited to, OASIS accuracy, productivity, case mix, laptop transfers, etc.
• Participates in primary on-call, weekend and holiday on-call, and other assignments based on agency needs.
• Effectively supervise LPN and LNA staff as mandated by State and Federal agencies required.
