Dartmouth Health Home Care is seeking an RN Case Manager to work full-time or part-time in Upper Valley, Vermont. You will screen, evaluate, and manage patient care plans in collaboration with the health care team and the patient and family, ensuring high-quality home health services.
A compact license or VT/NH licenses are required, with BLS within 90 days of hire. The role offers a broad benefit package within the Dartmouth Health system, with a day shift and on-call requirements to support

The RN Case Manager at the direction of the Clinical Manager, screens, evaluates, re-evaluates and treats patients referred by a licensed provider. Coordinates and communicates treatment activities with health care team members and the patient. Works closely with the interdisciplinary team to ensure high-quality patient care.

– Must hold at least one active unencumbered RN license in either unencumbered VT or NH at the time of hire. Employees must have active unencumbered RN licenses in both VT and NH within 30 days of their hire date. – Valid driver’s license with a clean driving record – Current car insurance, which meets minimum standards – BLS within 90 days of hire
