Dartmouth Health is seeking a Nursing professional to coordinate and manage a home-based hospice caseload under the Clinical Manager. You will assess, plan, implement, and evaluate care with the interdisciplinary team, supervise Home Health Aides, and ensure timely, quality patient outcomes in Vermont and New Hampshire.
Requirements include an active RN license in VT/NH or a compact license, graduation from an accredited nursing program, and preferred hospice/homecare experience.
At the direction of the Clinical Manager, coordinates/manages a case load comprised of Hospice patients. Assesses, plans, implements, coordinates, and evaluates care for each patient/family in conjunction with the hospice interdisciplinary team. Case management includes but is not limited to; comprehensive needs assessments, development of appropriate plan of care, referral to internal and external resources, evaluation of outcomes, and provision of nursing educational, clinical and related services. Supervises the Licensed Nursing Assistants/ Home Health Aides.
