Job summary

Dartmouth Health Home Care is seeking an RN Case Manager to work full-time or part-time in Upper Valley, Vermont. You will screen, evaluate, and manage patient care plans in collaboration with the health care team and the patient and family, ensuring high-quality home health services.

A compact license or VT/NH licenses are required, with BLS within 90 days of hire. The role offers a broad benefit package within the Dartmouth Health system, with a day shift and on-call requirements to support

Qualifications

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school of nursing
• One year of experience as an RN in a home health care setting is preferred

Responsibilities

• Screens, evaluates, re-evaluates and treats patients referred by a licensed provider.
• Coordinate the patient’s care plan through interdisciplinary communication and case conferences.
• Provide ongoing assessment of the patient’s clinical, functional, and emotional status.
• Document evaluations, treatment goals, and objectives.
• Teach self-care techniques and provide health counseling to patients and caregivers.
• Provide clinical oversight of LPNs and LNAs in the home.
