Job summary

UnitedHealth Group's Optum unit is seeking a Nurse Case Manager in Savannah, GA. You will guide patients and families through the care continuum, assess needs, coordinate care, support discharge and transition planning, and manage ongoing outpatient care under supervision.

You will help ensure patients receive appropriate care, provide education, coaching, and treatment-decision support, and collaborate with a Care Advocate and Social Worker.

Qualifications

• Current RN license, specific to assigned state(s).
• 1+ year of clinical RN experience.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates to develop and implement a care plan.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health through teaching and rehabilitation.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
