Amedisys, part of the Optum family of businesses, seeks a Nurse Case Manager in Alabama to guide patients and families through the care continuum. Under supervision of an RN or MD, you will assess needs, coordinate care, support discharge and transition planning, and manage ongoing outpatient care.

You will collaborate with a Care Advocate and Social Worker, perform patient assessments, supervise LPNs and HHAs, complete documentation timely, and participate in ongoing clinical development.

Qualificações

• Current RN license
• 1+ years of clinical RN experience
• Current CPR certification
• Valid driver’s license, reliable transportation and liability insurance

Responsabilidades

• Assess patients and develop a care plan with the care team
• Coordinate referrals and document rationale
• Educate patients on self-care and rehabilitation
• Supervise LPNs and HHAs
• Complete timely, accurate documentation
• Communicate patient progress to clinical manager
• Plan and provide staff education
• Participate in on-call duties
• Engage in clinical development and continuing education
• Perform other assigned duties

Conhecimentos

RN clinical experience

CPR certification
