Job summary

Amedisys, part of the Optum family, is hiring a Nurse Case Manager to guide patients and families through the care continuum, coordinating care and supporting discharge planning. You will work under supervision of an RN or MD, providing education and decision-support while collaborating with a Care Advocate and Social Worker.

Primary responsibilities include patient assessments, care planning, appropriate referrals, and timely documentation, with opportunities for ongoing clinical development.

Qualifications

• Current RN license in state(s) assigned.
• CPR certification.
• 1+ year of clinical RN experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess patient needs and coordinate care plan with care team.
• Make referrals as indicated and document rationale.
• Educate patients and promote self-care techniques.
• Supervise LPNs and HHAs.
• Maintain timely and accurate documentation.
