Job summary

Amedisys, part of the Optum family, is seeking a Nurse Case Manager to guide patients and families through the care continuum, coordinating care and supporting discharge planning. Under supervision of an RN or MD, you will educate, coach, and support treatment decisions while collaborating with Care Advocates and Social Workers.

You will assess needs, coordinate outpatient care, supervise clinical staff, and participate in ongoing education.

Qualifications

• Current RN license for the applicable state(s).
• Current CPR certification.
• 1+ year of clinical RN experience.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess patients and develop care plans with the care team.
• Refer to other disciplines as needed with rationale.
• Educate patients and promote self-care practices.
• Supervise LPNs and HHAs.
• Complete timely and accurate documentation.
• Communicate patient progress to the clinical manager.
• Plan and provide staff education.
• Perform on-call duties as assigned.
• Participate in clinical development and ongoing education.
• Perform other duties as assigned.
