Amedisys, part of the Optum family of businesses, is seeking a Nurse Case Manager to guide patients and families through the care continuum. You will assess needs, coordinate care, support discharge planning, and manage ongoing outpatient care under supervision of an RN or MD.

Responsibilities include conducting patient assessments, coordinating referrals, teaching self-care techniques, supervising LPNs and HHAs, and ensuring timely documentation.

Formación

• Current RN license in the assigned state(s).
• Valid driver’s license, reliable transportation, and liability insurance.
• Current CPR certification.
• 1+ year of clinical RN experience.

Responsabilidades

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
• Promotes patient health through teaching and rehabilitative measures; assists patients in learning self-care techniques.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
