Job summary

Qualifications

• Current RN license applicable to the state(s) of assignment.
• Current CPR certification.
• 1+ year of clinical experience as an RN.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitative measures; assists patients in self-care.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
