Job summary

UnitedHealth Group is seeking a Registered Nurse (RN) Case Manager to guide patients and families through the care continuum, coordinating care, supporting discharge planning, and managing outpatient care. Under supervision of an RN or MD, you will educate, coach, and assist with treatment decisions while collaborating with a Care Advocate and Social Worker.

You will perform assessments, plan care, refer to other disciplines as needed, and supervise LPNs and HHAs.

Qualifications

• Current RN license in the state(s) you work.
• 1+ year of clinical RN experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and insurance.

Responsibilities

• Performs patient assessments and coordinates a plan of care with the care team.
• Refers to other disciplines as needed and documents rationale if not.
• Promotes patient health through teaching and rehabilitation.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities as assigned.
• Participates in clinical development and continuing education.
