Job summary

Team Select Home Care is seeking an experienced Registered Nurse Case Manager in Texas to oversee patient care plans, coordinate services with physicians and staff, and ensure compliance with state requirements. You will perform initial evaluations, manage medications, and educate patients and families on home health strategies.

The role requires strong communication, critical thinking, and the ability to work independently while supporting a multidisciplinary team within an exempt, on-call

Qualifications

• Graduate of an approved school of professional nursing
• Licensed in state(s) practicing

Responsibilities

• Develops, delivers/implements, revises, and evaluates individualized patient-centered care plans under physician supervision
• Provides case management to assigned patients and staff
• Completes required data collection and documentation within required timeframes
• Makes initial evaluation visits and reevaluates nursing needs
• Coordinates care among patients, families, and providers
• Administers medications per policy and orders
• Attends case conferences and reports changes in condition
• Educates patients and families on home health strategies
• Maintains confidentiality and adheres to billing and regulatory requirements
• On-call duty nights, weekends, and holidays as assigned
• Performs other duties as assigned
