Job summary

Team Select Home Care seeks an experienced Case Manager RN to oversee patient care plans, coordinate with physicians, families, and interdisciplinary staff in a home health setting.

You will assess nursing needs, manage caseloads, ensure accurate documentation, and participate in quality improvement activities. A current RN license and CPR certification are required, with at least one year of nursing/home health experience.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• Minimum (1) year nursing experience required; prior home health experience preferred.

Responsibilities

• Develops, delivers/implements, revises, and evaluates individualized patient-centered care plans under physician supervision.
• Provides case management to an assigned group of patients and ancillary staff.
• Submits required data collection sets (OASIS, PAR) and other documentation within required timeframes.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medication per agency policy and physician orders.
• Communicates changes in patient condition to physician and team.
