Job summary

Integritus Healthcare in Massachusetts is seeking a dedicated RN Case Manager to deliver comprehensive home health services, including Oasis assessments, plan of care development, and coordination with primary care physicians.

The role requires 1–2 years in health care, current CPR certification, and Massachusetts RN licensure. This full-time position offers a competitive salary and benefits, with on-site/home-based care delivery and on-call responsibilities.

Qualifications

  • Minimum 1–2 years experience in health care.
  • Experience in home health preferred.
  • Associate's degree in nursing is required.
  • Bachelor’s degree in nursing is preferred (not needed).
  • Current CPR Certification.
  • Massachusetts licensure: Registered Nurse.
  • Valid driver’s license.

Responsibilities

  • Assesses patient/family learning needs and tailors information delivery.
  • Establishes goals and develops individualized plans of care with patients and families.
  • Collaborates with healthcare providers and community resources for care planning and discharge.
  • Follows physician orders in accordance with patient care policies.
  • Reconciles medications with patient and physician.
  • Manages caseload within the team care model.
  • Creates daily work plans based on priorities.
  • Triages patient visits and calls by priority.
  • Demonstrates strong assessment and care planning skills.
  • Supervises Home Health Aides as required.
  • Maintains knowledge of pharmacology and medication reconciliation.
  • Communicates with all disciplines in home health care.

The RN Case Manager manages and delivers comprehensive home health services, including Oasis assessments, Plan of Care development, goal development and interventions, to patients within their place of residence in collaboration with primary care physicians. Additional duties may include telephone triage, problem‑solving, patient/caregiver/family education, advocacy, and support.

Job Duties
  • Assesses patient/family learning needs, style and limitations and adjusts the delivery of information.
  • Establishes realistic goals and develops individualized plans of care in collaboration with the patient, family and members of the health care team.
  • Collaborates with patient/family and other healthcare providers and/or community resources with planning of care and discharge.
  • Follows physician orders on all patients assigned in accordance with patient care policies.
  • Reconciles medications with patient and physician consistently.
  • Effectively manages assigned caseload, within the team model of care delivery.
  • Establishes a daily work plan based upon patient/family priorities of service and total area needs.
  • Promptly triages patient visits, messages, and phone calls according to priority and urgency.
  • Demonstrates excellent physical assessment and care planning skills.
  • Supervises HHA in accordance with state and federal requirements.
  • Demonstrates knowledge of pharmacology and medication administration and reconciliation.
  • Communicates and collaborates with all disciplines in the home care setting on a regular basis or immediately if there are any critical needs or crisis interventions.
  • Completes documentation accurately and timely.
  • Performs on‑call responsibilities and on‑call services to patients and families assigned.
  • Participates in agency quality management and safety activities.
  • Participates in ongoing staff meetings and in‑services.

This is a full‑time, 40‑hour‑per‑week position. Yearly Salary Range (based on years of experience) – $94,000 – $99,000.

Qualifications

(Minimum qualifications will be considered required unless specifically stated otherwise)

Experience
  • Minimum of one (1) to two (2) years of experience in health care.
  • Experience in home health preferred.
Education And Training
  • An associate’s degree in nursing is required.
  • A bachelor’s degree in nursing is preferred. (not needed)
License, Certification & Registration
  • Possesses and maintains current CPR Certification.
  • Current Massachusetts licensure: Registered Nurse.
  • Valid driver’s license.
