Job summary

IntegriHome, Inc. in East Longmeadow, MA, is hiring a full-time Certified Nursing Assistant. This role offers a starting base rate of $18.00–$22.09/hr, with weekly pay and comprehensive benefits.

The CNA will join a collaborative team within one of the region's large post-acute care networks, dedicated to high-quality, compassionate patient care. The position emphasizes personal care, observation, and documentation, working closely with RNs and therapists to support patients from post-acute

Qualifications

• CNA certification required or equivalent state credential.
• Ability to perform personal care tasks with dignity and respect.
• Good communication with patients, families and supervising RNs.

Responsibilities

• Perform personal care tasks as assigned and respond to patient requests promptly.
• Ensure patient safety and use equipment correctly.
• Provide baths, oral hygiene, hair care, and linen changes as directed.
• Assist with dressing and undressing of patients.
• Monitor skin condition and report changes to RN.
• Observe status changes of patients and communicate with team.
• Assist in ambulation and prescribed exercises.
• Support self-administration of medications as allowed by state regulations.

Position Summary: This is a full-time, 32-hour position. The Certified Nursing Assistant is a paraprofessional member of the Interdisciplinary
Team who performs various services for a patient and family as necessary to meet the patient's personal needs.
T h e C N A is responsible for observing, reporting, and documenting care performed.

Essential Job Functions

• Responsible for performing personal care needs after being given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the RN immediately.
• Responsible for meeting the safety needs of patients and using equipment safely and properly
• Responsible for giving direct personal care, including complete or partial baths in the tub or shower as ordered, back rubs, oral hygiene, hair care, shaving (using an electric razor only), and changing bed linen as often as assigned.
• Responsible for assisting in the dressing and undressing of patients as assigned.
• Responsible for providing proper care and observation of the patient’s skin to prevent breakdown of tissue over bony prominence.
• Responsible for reporting on the patient's condition and significant changes to the assigned RN
• Responsible for being aware of and reporting on the status changes of the caregiver or other individuals living with the patient, and interpersonal issues that may come up
• Assist in ambulation and exercise as instructed by the RN or therapist while adhering to the transfer/assist transfer policy of the agency.
• Assistance with self-administered medications as allowed by state regulations ('reminders.
