Job summary

Mercy is seeking a Per Diem Registered Nurse to provide day-to-day patient care in the home setting across varying acuity. Responsibilities include case management, admissions, post-hospitalization assessments, and skilled nursing tasks performed per policy and procedure.

You will document care using Epic, maintain safety, and collaborate with a caring team. CPR certification and current state licensure are required; one year of related home care experience is preferred, as is a valid driver's

• Current state licensure is required.
• CPR certification is required.

Responsibilities

• Provide day-to-day patient care in the home setting with varying acuity.
• Case management and total care of multiple patients; admissions, post-hospitalization assessments, recertifications and discharges.
• Patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing per policy and procedure.
• Documentation of care using the Epic system.

Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.

Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

Position Details:

Education: Requires graduation from accredited nursing school.
