Job summary

Mercy is seeking a Per Diem Registered Nurse to provide day-to-day home health care to patients in Oklahoma. Responsibilities include case management, admissions and discharges, post-hospital assessments, IV therapy, wound care, and patient teaching.

Documentation is completed using Epic per Mercy standards. The role requires graduation from an accredited nursing school, current Oklahoma licensure, and CPR certification, with a preference for at least one year of related experience.

Qualifications

• Graduation from an accredited nursing program is required.
• Current state nursing licensure is required.
• CPR certification is required.
• One year of related field experience is preferred.

Responsibilities

• Provide day-to-day home care and skilled nursing to patients with varying acuity.
• Perform admissions, post-hospitalization assessments, recertifications, and discharges.
• Provide patient teaching, IV therapy administration, wound care, and other duties per policy.
• Document care using Epic in a timely and accurate manner.
• Per diem visits are scheduled as needed following department policies.

Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.

Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

Position Details:

Education: Requires graduation from accredited nursing school.
Licensure: Current state licensure. Valid driver's license.
