Job summary

Mercy is seeking a Per Diem Registered Nurse for home health in Oklahoma. You will provide day-to-day patient care in patients’ homes, perform admissions and post-hospital assessments, and manage recertifications and discharges with precision.

You will also educate patients and administer IV therapy as needed, while documenting all care in Epic. A valid RN license and CPR certification are required, along with a driver’s license for travel.

Qualifications

• Graduation from accredited nursing school is required.
• Current state RN licensure is required.
• CPR certification is required.
• Valid driver’s license is required.

Responsibilities

• Provides day-to-day patient care in the home setting, including case management and total care for multiple patients.
• Performs admissions, post-hospitalization assessments, recertifications and discharges.
• Delivers patient teaching and specialized nursing tasks per policy, including dressing changes and IV therapy.
• Maintains accurate documentation using the Epic system.

Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.

Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

Position Details:

Education: Requires graduation from accredited nursing school.
Licensure: Current state licensure. Valid driver's license.
