Job summary

Mercy is seeking per diem home health nurses to provide day-to-day patient care in the home setting. You will manage multiple patients with varying acuity, perform admissions and post-hospitalization assessments, and document in Epic according to policy.

The role emphasizes compassionate care, adherence to Mercy standards, and collaboration with a caring team to support patients safely at home.

Qualifications

• Graduation from an accredited nursing school is required.
• Current state nursing licensure and CPR certification are required.
• One year of related home health experience is preferred.

Responsibilities

• Provide day-to-day patient care in the home setting.
• Perform case management and total care of multiple patients with varying acuity.
• Conduct admissions, post-hospitalization assessments and recertifications.
• Document appropriately in Epic and follow Mercy policies.

Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.

Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

Position Details:

Education: Requires graduation from accredited nursing school.
Licensure: Current state licensure. Valid driver's license.
