Job summary

Dartmouth Health Home Care is seeking an RN Case Manager to join our care team in the Upper Valley, VT/NH, delivering home health and hospice services with day shifts and on-call as needed. This full-time/part-time role offers a $10,000 New Hire Retention Bonus, extensive benefits, and opportunities for professional growth.

Qualifications

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school of nursing.
• One year of experience as an RN in a home health care setting is preferred.

Responsibilities

• Screens, evaluates, re-evaluates and treats patients referred by a licensed provider.
• Coordinates and communicates treatment activities with health care team members and the patient.
• Develops a care plan in collaboration with the patient’s physician, the patient, and their family.
• Periodically reviews and re-certifies the patient care plan as appropriate.
• Coordinates the patient’s care plan through interdisciplinary communication and case conferences.
• Provides ongoing assessment of the patient’s clinical, functional, and emotional status.
• Documents evaluations, treatment goals, and objectives.
• Teaches the patient and caregiver self-care techniques as appropriate.
• Provides clinical oversight of LPNs and LNAs in the home.

Responsibilities

The RN Case Manager at the direction of the Clinical Manager, screens, evaluates, re-evaluates and treats patients referred by a licensed provider. Coordinates and communicates treatment activities with health care team members and the patient. Works closely with the interdisciplinary team to ensure high-quality patient care.

1. Develops a care plan in collaboration with the patient’s physician, the patient, and their family, or the primary caregivers upon admission.
2. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
3. Coordinate the patient’s care plan through interdisciplinary communication and case conferences, as appropriate. Advise other disciplines of discharges in a timely manner.
4. Provide ongoing assessment of the patient’s clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patient’s physician and DHHC colleagues.
5. Document evaluations, treatment goals, and objectives.
6. Teach the patient and caregiver self-care techniques as appropriate. Provide medication, diet, and other instructions as ordered by the provider and utilizes opportunities for health counseling with patients and caregivers.
7. Provides clinical oversight of LPNs and LNAs in the home.

Qualifications

Required Licensure/Certifications

• Must hold at least one active unencumbered RN license in either unencumbered VT or NH at the time of hire. Employees must have active unencumbered RN licenses in both VT and NH within 30 days of their hire date.
• Valid driver’s license with a clean driving record
• Current car insurance, which meets minimum standards
• BLS within 90 days of hire
