Dartmouth Health Home Care is hiring an RN Case Manager to support our comprehensive home health and hospice services in Upper Valley and surrounding areas. You will collaborate with a multidisciplinary team to plan, implement, and adjust patient care plans, ensuring high-quality outcomes and safety for patients in their homes.
You must hold active RN licenses in VT/NH or a compact license, have a nursing degree, and ideally one year of home health experience.

The RN Case Manager at the direction of the Clinical Manager, screens, evaluates, re-evaluates and treats patients referred by a licensed provider. Coordinates and communicates treatment activities with health care team members and the patient. Works closely with the interdisciplinary team to ensure high-quality patient care.
