Job summary

Dartmouth Health Home Care is seeking an RN Case Manager to deliver skilled nursing and supportive services in patients' homes within Vermont. You will coordinate with the interdisciplinary team to create and revise patient care plans, provide education, and ensure high-quality care on day shifts with potential on-call requirements.

Candidates must hold VT or NH RN licensure (or a compact license) and have current BLS.

Qualifications

• Must hold an active RN license in VT or NH or a compact license.
• Graduate of an accredited nursing program.
• BLS certification within 90 days of hire.

Responsibilities

• Develops and tunes the patient care plan with the physician, patient and family.
• Reviews and recertifies the care plan as needed.
• Coordinate care through interdisciplinary communication and case conferences.
• Assess clinical, functional and emotional status and report changes.
• Document evaluations and treatment goals.
• Educate patient and caregiver on self-care and care instructions.
• Provide clinical oversight of LPNs and LNAs in the home.

Responsibilities

The RN Case Manager at the direction of the Clinical Manager, screens, evaluates, re-evaluates and treats patients referred by a licensed provider. Coordinates and communicates treatment activities with health care team members and the patient. Works closely with the interdisciplinary team to ensure high-quality patient care.

1. Develops a care plan in collaboration with the patient’s physician, the patient, and their family, or the primary caregivers upon admission.
2. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.
3. Coordinate the patient’s care plan through interdisciplinary communication and case conferences, as appropriate. Advise other disciplines of discharges in a timely manner.
4. Provide ongoing assessment of the patient’s clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patient’s physician and DHHC colleagues.
5. Document evaluations, treatment goals, and objectives.
6. Teach the patient and caregiver self-care techniques as appropriate. Provide medication, diet, and other instructions as ordered by the provider and utilizes opportunities for health counseling with patients and caregivers.
7. Provides clinical oversight of LPNs and LNAs in the home.

Qualifications

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school of nursing
• One year of experience as an RN in a home health care setting is preferred

Required Licensure/Certifications

• Must hold at least one active unencumbered RN license in either unencumbered VT or NH at the time of hire. Employees must have active unencumbered RN licenses in both VT and NH within 30 days of their hire date.
• Valid driver’s license with a clean driving record
• Current car insurance, which meets minimum standards
• BLS within 90 days of hire
