Boost Home Healthcare - Greater Boston is seeking a compassionate Registered Nurse to join as a part-time Home Health Case Manager in Massachusetts. You will manage a caseload, perform OASIS assessments, and coordinate with physicians, therapists, and social workers to ensure timely, high-quality home care.

Qualifications

• RN with MA state license is required.
• At least 1 year of home health or case management experience preferred.
• Experience administering IVs and medications.
• Strong critical thinking, communication, and time management skills.
• Ability to work independently and manage a caseload.

Responsibilities

• Manage the Care Journey: primary contact for a caseload of home health patients and coordinate services.
• Conduct OASIS assessments and develop individualized care plans with the interdisciplinary team.
• Coordinate services with physicians, nurses, therapists, social workers, and home health aides.
• Monitor patient progress, update care plans, and communicate with families and caregivers.
• Maintain accurate and timely clinical documentation per Medicare guidelines.
• Advocate for patients by providing education and resources to promote independence.
