Right at Home Boston Northwest is seeking a compassionate Registered Nurse (RN) / Case Manager to join our new Hyannis office. You will conduct in-home assessments, manage care plans, and support caregivers while building relationships with clients, families and referral partners.

Competitive Hourly Pay, Incentive & Benefits
$45-$55 per hour, based on experience, plus a performance-based incentive with additional upside as the Cape Cod office grows.

Both part-time and full-time schedules are available, generally 24-40 hours per week. We will work with you to find an arrangement that supports you and our clients.
New-Office Impact With Established Support
Receive initial training and guidance from experienced nursing leadership, with room to take on broader clinical responsibility as the office grows if that is a path that interests you.
Conduct in-home assessments and reassessments and develop or update individualized care plans.
Provide ongoing clinical case management, nursing supervision, follow-up visits, and timely documentation.
Support, educate and coach caregivers; respond to changes in condition and help ensure care plans are followed.
Serve as a trusted clinical liaison for clients, families, caregivers, physicians, and other healthcare professionals.
Partner with the established scheduling, care management, and operations teams to support appropriate staffing, continuity of care, and strong day-to-day operations.
Build relationships with hospitals, rehabilitation centers, senior living communities, Aging Services Access Points, physicians, and other Cape Cod referral partners.
Represent Right at Home professionally in the community, support smooth care transitions, and contribute to the responsible growth of the new office.
Active, unrestricted Registered Nurse (RN) license in Massachusetts.
Two or more years of nursing experience; home care, geriatrics, private-duty care, or care-management experience strongly preferred.
Strong clinical judgment, communication, organization, relationship-building, and documentation skills.
A self-directed, collaborative professional who is comfortable taking initiative, adapting as the office grows, and developing strong referral relationships.
Valid driver's license, insured vehicle, and ability to travel throughout Cape Cod.
